Oral, Dental and Maxillofacial Surgery – Ümraniye, Istanbul

What Is Oral, Dental and Maxillofacial Surgery?

In our Oral, Dental and Maxillofacial Surgery department, we diagnose and surgically treat all kinds of conditions and diseases of the teeth and the soft tissues of the mouth. Surgical procedures are performed in a sterile environment to support successful outcomes.

One of the most common surgical procedures at our clinic is the extraction of wisdom teeth. We remove fully or partially impacted wisdom teeth that cannot erupt due to lack of space in the jaw or poor positioning, with close attention to the patient’s comfort. This is followed by the surgical removal of cysts found at the root tips of teeth that have had root canal treatment. We also routinely perform surgical procedures such as removing larger cysts, lengthening teeth that have become too short to receive a porcelain crown, and preparing the jawbone for prostheses or implants.

Impacted Tooth and Wisdom Tooth Surgery

Impacted tooth surgery is the procedure used to remove teeth that are impacted, that is, trapped within the gum or bone.

Impacted tooth surgery includes procedures such as:

  • Impacted wisdom teeth.
  • Canine teeth.
  • Molars.
  • Extraction of supernumerary (extra) teeth

Tooth Root Cyst

Tooth root cysts can develop as a result of tooth decay and untreated decayed teeth. Cysts can also form at the root tips of teeth with faulty or incomplete root canal treatment. In addition, some people may develop cysts due to a genetic predisposition. A cyst at the tooth root is surrounded by a membrane and contains semi-fluid material and air; this fluid-filled tissue contains pathogens. Because a developing cyst often causes no symptoms in the area where it forms, it may go unnoticed.

A cyst at the tooth root develops as one of the body’s defense mechanisms against harmful factors. At first, the cyst is very small and is not felt in the area where it is located. Later, it grows as it fills with more fluid and pathogens. As it grows, the cyst puts pressure on the surrounding tissues, causing pain. This pressure can also cause a feeling of numbness, which at times may be accompanied by pain in the lip and cheek area.

A cyst at the tooth root must be treated. If left untreated, it can lead to serious complications.

Dental Cyst Treatment

Treatment is planned according to the type of cyst in the tooth. The first step is to dry up the infection in the tooth. Infection can also cause severe pain, so the patient is given antibiotics for a period of time. The dentist then thoroughly cleans the area of the abscess with instruments, and root canal treatment is performed on the tooth. In root canal treatment, the tooth is accessed through a small opening, the nerves inside are removed and the tooth is sealed. In this way, the tooth can be preserved in the mouth.

When Should Wisdom Teeth Be Removed?

Wisdom teeth often do not fully erupt, or remain impacted at an angle, because there is not enough space in the jaw. Extraction is recommended in situations such as recurring gum inflammation (pericoronitis), pain, decay in the neighboring tooth, cyst formation or planning for orthodontic treatment. Fully erupted wisdom teeth that cause no problems can remain in the mouth with regular check-ups. The decision is made after examination and evaluation with a panoramic X-ray or CT scan.

Apical Resection (Root-End Surgery)

For root-tip infections that do not heal despite root canal treatment, the infected tissue and cyst at the root tip are surgically removed without extracting the tooth. This procedure offers a chance to save the tooth and is usually planned as a continuation of endodontic treatment.

Bone Grafting and Sinus Lift Before Implants

After tooth loss, the jawbone gradually shrinks over time. When bone volume is insufficient, bone grafting can be performed before implant treatment. If the sinus cavity is very close to the bone in the upper back region, the sinus floor is raised (sinus lift) to create enough space for the implant.

Other Surgical Procedures

  • Frenectomy: Correction of a lip or tongue tie that is short or thick
  • Gum surgery: Flap surgery and gum grafting in advanced gum disease
  • Biopsy: Taking a sample from suspicious lesions in the mouth for diagnosis
  • Complex extractions: Extraction of broken teeth, teeth with curved roots or teeth fused to the bone

What to Keep in Mind After Surgery

  • Keep biting on the gauze pad for 30 to 45 minutes
  • For the first 24 hours, avoid spitting, drinking through a straw and gargling
  • On the first day, applying cold compresses to the outside of the face at intervals helps reduce swelling
  • In the first few days, eat lukewarm, soft foods and do not smoke
  • Take the medications recommended by your dentist as directed and attend your follow-up appointment

Frequently Asked Questions About Oral and Dental Surgery

Is wisdom tooth extraction painful?

The procedure is performed under local anesthesia, so the area is numb during the extraction. There may be mild pain and swelling for a few days afterwards, which can be managed with the recommended medications.

How long does wisdom tooth extraction take?

Extracting an erupted wisdom tooth usually takes a short time. For impacted teeth, the procedure may take an average of 20 to 60 minutes, depending on the position of the tooth.

How long does swelling last after an extraction?

After impacted tooth extractions, swelling usually peaks on the 2nd or 3rd day and decreases noticeably within a week.

When can I eat after a tooth extraction?

Once the numbness wears off, you can eat lukewarm, soft foods. On the first day, hot, hard and spicy foods should be avoided.

What happens if a dental cyst is not treated?

Untreated cysts can grow over time, erode the surrounding bone, damage neighboring teeth and lead to infection. For this reason, early diagnosis and treatment are important.
